Our office requires a referral and relevant medical records before scheduling a new patient appointment. Please have your referring provider fax this information to us prior to calling.

For patients referred for osteoporosis, we require the following documents to schedule your appointment:

• Most recent bone density report with images
• Referral from your provider
• Office visit notes
• Recent lab results
